when cancelling medical form creation.... Empty when cancelling medical form creation....

we are taken back to the list page supposedly, but the medical form data is displayed horizontally.
it seems to pull the medical form data and display it horizontally.

Ah yes - a known issue. That's actually a list of the medical forms (for everyone I believe). I don't even think we really need this view.
